The CDL offers a variety of clinical services designed to help individuals and their families identify developmental levels and learning styles that will enable them to be successful in life. In order to schedule an appointment, please follow Steps 1 and 2:
The Contact Form is reviewed by a CDL clinician for appropriateness. If our services match the identified needs, a referral packet requesting additional information will be mailed to you. |
After all of the requested information and questionnaires are returned, our clinicians will review the information to determine the individual's evaluation needs. Then the individual is placed on a CDL waiting list and a letter is sent to confirm that the information has been received and the individual was placed on a waiting list. |
While we are eager to help you and your family, the length of our waiting list varies and can be extensive, depending on the individual's evaluation needs. Therefore, there is a possibility that we may not be able to evaluate every individual referred to the CDL as quickly as we would like. In this event, you may want to contact your primary care physician or school about evaluation alternatives or check out our list of resources while you are waiting.
